Free Delivery Note Template: Excel, Word, PDF

A delivery note with an item table and a parcel box

A delivery note goes with the goods when a shop sends them: what was sent, how many, to whom, and a line for the receiver to sign that everything arrived. Shops use it with company customers, large deliveries and credit sales paid later. Download the template below for free, no sign-up needed.

Delivery note, invoice and receipt: the difference

  • Delivery note: goes with the goods, as proof of what was sent and that the receiver got it all
  • Invoice: asks for payment, with a due date. See the free invoice template
  • Receipt: issued after the money arrives. See the free receipt template

A delivery note is a business document, not a tax document. A shop that is not VAT registered can use this template as it is, and must not call it a tax invoice.

Can one document be both the delivery note and the tax invoice?

Yes, for a VAT-registered shop in Thailand. The Revenue Department lets a tax invoice be combined with other business documents, such as a delivery note or an invoice, in one set, as long as it carries every item a full tax invoice needs:

  • the words 'tax invoice' where they are easy to see
  • the seller's name, address and tax ID
  • the buyer's name and address
  • the tax invoice number, and the book number if there is one
  • the name, kind, type, quantity and value of the goods
  • the VAT, shown apart from the value of the goods
  • the date the tax invoice is issued
  • any other wording the Director-General of the Revenue Department sets

If the tax invoice is not the first sheet of the set, it must say 'document issued as a set'. The items can be in Thai or English, but amounts must be in baht.

For an ordinary sale of goods, VAT is due when the goods are delivered, or earlier if the shop is paid, ownership passes or the tax invoice is issued first. That is why VAT-registered shops often issue one delivery note and tax invoice when the goods go out.

The files on this page are a delivery note only. A VAT-registered shop that wants to use it as a tax invoice too should have an accountant check the items first.

What a delivery note needs

  • Your shop: name, address, phone, LINE OA or email
  • The document: delivery note number, delivery date, and the purchase order or order number it refers to
  • The receiver: customer name, delivery address and phone
  • The items: item code, name, quantity and unit; prices are optional
  • How it was sent, and the tracking number if a courier carried it
  • Signature lines for the sender and the receiver, with the date received

How to fill in a delivery note, step by step

  1. Add your shop name, address and contact details
  2. Give the delivery note a number that never repeats, such as DN-2026-0035, and add the delivery date
  3. Add the customer's purchase order number or your order number, so the documents can be matched
  4. Add the receiver's name, delivery address and phone
  5. Add the items one per line with quantity and unit; count the real goods before packing
  6. Add how it was sent and the tracking number; if you deliver it yourself, write who delivered it
  7. Print two copies: one for the customer, and one the customer signs for you to keep

Example: a delivery to a company customer

This example uses made-up data. A shop delivers 40 embroidered T-shirts, 40 printed mugs and 40 tote bags to a company customer against purchase order PO-0187.

  • 3 items, 120 pieces in total
  • The customer signs and dates the copy the shop keeps
  • The shop sends the invoice afterwards, quoting this delivery note number

Send the delivery note in chat

  • Take a photo or scan of the copy the customer signed and send it in the customer's chat, to keep as proof
  • If the customer says something is missing, check the delivery note right away for what was sent
  • If the price is not agreed yet, start with a quotation: see the free quotation template
